APPLY

• Current Plum High Schiol Senior (GPA 3.5 or above)

• Copy of acceptance letter to college or trade school

• Two letters of recommendation (at least 1 from a teacher)

• The scholar-athlete will provide a brief essay. Minimum of one typed paragraph and maximum of one 8 1⁄2 x 11 typed page. The applicant will provide a brief essay about themselves and how their future plans or goals will contribute to the world we live in.

• Include name, date of birth, home address, email address and phone number

Please turn in the above materials to the Plum High School guidance office by April 1.

The winner will be awarded a $2500 scholarship.

Applications will be reviewed by Adam’s family for final determination.
